Where A Family Finds Their Team

Wraparound: Your Care Coordination Partner

We’re here to walk alongside your family.
When life feels overwhelming and your child is facing big challenges because of intellectual or developmental disabilities, we help you find a way forward — together.

Wraparound is all about building a support system that truly understands your family. We focus on your child’s strengths, listen to what matters most to you, and bring everyone involved in your child’s life onto the same page — all working toward your family’s goals.

We’ll help guide your family through tough times, make sure all your services are working together, and bring in anything else you need. Over time, you’ll feel more confident, more in control, and more supported.

Our process has four simple steps:

  1. Getting to Know You – We listen, build trust, and help your family picture a better future.

  2. Making a Plan Together – You decide what success looks like, and we build a team to get there.

  3. Doing the Work – We help you take steps toward your goals and track your progress.

  4. Moving Forward – As things improve, we step back and let your family take the lead.

With Wraparound, your family is never alone.

Mentorship: Personalized & Direct Support

Building Trust, Confidence, and Independence

When your child faces challenges due to intellectual or developmental disabilities, or is navigating difficult life circumstances, having a consistent, caring adult by their side can make all the difference.

Our mentorship program offers personalized, one-on-one support tailored to your child's unique needs and strengths. Mentors serve as trusted guides, helping youth develop essential life skills, build self-confidence, and work towards personal goals. Whether improving social interactions, exploring career interests, or learning daily living skills, our mentors are there every step of the way.

Research shows that mentorship can significantly benefit youth with disabilities, aiding in transitions to adulthood, enhancing self-esteem, and fostering independence. Programs like the Ready to Achieve Mentoring Program (RAMP) have successfully supported youth with disabilities through career-focused mentoring, leading to positive outcomes in employment and independent living.

Our mentors are trained to understand the complexities of each youth's situation, ensuring that the support provided is both compassionate and effective. By fostering a strong mentor-mentee relationship, we aim to empower your child to overcome challenges and achieve their fullest potential.
