Where A Family Finds Their Team
Wraparound: your care coordination partners
We’re here to walk alongside your family.
When life feels overwhelming and your child is facing significant challenges, we help discover a new path forward together.
Wraparound is all about building a support system that truly understands your family. We focus on your child’s strengths, listen to what matters most to you, and get everyone involved in your child’s life onto the same page — all while working on your goals.
We’ll help guide your family through tough times, ensure all your services work together, and bring anything else you need into your care process. Over time, we find that families feel more confident, in control, and supported.
Our process has four simple steps:
Getting to Know You – We listen, build trust, and help your family picture a better future.
Making a Plan Together – You decide what success looks like, and we build a team to get there.
Doing the Work – We help you take steps toward your goals and track your progress.
Moving Forward – As things improve, we step back and let your family take the lead.
With Wraparound, your family is never alone.
All the services we offer are free to youth and families on Medicaid and the CHRP waiver.
Mentorship: direct one-on-one support
Building Trust, Confidence, and Independence
When your child faces challenges or is navigating difficult life circumstances, having a consistent, caring adult by their side can make all the difference.
Our mentorship program offers personalized, one-on-one support tailored to your child's unique needs and strengths. Mentors serve as trusted guides, helping youth develop essential life skills, build self-confidence, and work towards personal goals. Whether improving social interactions, exploring career interests, or learning daily living skills, our mentors walk alongside your child every step of the way.
Mentorship can significantly benefit youth with disabilities, aiding in transitions to adulthood, enhancing self-esteem, and fostering independence. Programs like ours have successfully supported youth with disabilities through goal-focused mentoring, leading to positive outcomes in employment and independent living.
Our mentors are trained to understand the complexities of each youth's situation, ensuring that the support provided is both compassionate and effective. By fostering a strong mentor-mentee relationship, we aim to empower your child to overcome challenges and achieve their fullest potential.

Vision
We envision the lived experiences of youth across Colorado who have been struggling to turn their situations around, and find places of thriving in their homes and communities.
We envision a future of revitalized community health, bringing together all possible supports to bolster a child’s success.
We also envision that the services we offer can properly bring together previously disjointed pieces of an at-risk youth’s life, thereby bolstering both internal and external health.
Mission
We are driven to enhance the outcomes of all the youth we serve towards the highest potential for success through equitable service.
We seek to provide high-quality, metric-driven value in the lives of the families we work for.
We have chosen wraparound facilitation, mentorship, and our parent support group as the conduits to make all of this happen for youth and families across Colorado.
